Forms and Information formatted for Printing
| ** To encourage all units to use the online registration process starting in 2011 an
additional fee will be added:

| $4.00 for EACH unit registration form mailed in for the P2E staff to enter and NOT entered using the online process.
| |
| $3.00 for EACH individual registration form mailed in for the P2E staff to enter and NOT entered using the online process.
| |
The additional fee MUST be included with registration forms that are mailed before the P2E staff
will enter the unit or individual information. If the additional fee is not enclosed the forms will be returned to the sender.
Note: This fee and/or payment will NOT show up anywhere in the registration screens.
